Gastrointestinal Leiomyosarcoma



Abstract
This paper is an analysis of seventeen cases of gastrointestinal leiomyosarcoma who underwent surgical intervention for the period from 1983 to 1992, and the results were summarized as follows:
1) The age was ranged from 17 years to 70 years(mean: 51 years) and the most prevalent age group was 6th and 7th decades(70.6%). The sex ratio of male to female was 1 : 1.2.
2) The primary sites were stomach(6 cases), duodenum(3), jejunum(3). Ileum(3) and rectum(2).
3) The main symptoms and signs were abdominal pain, mass and discomfort, and the other minor symptoms were as melena, anemia, weight loss and high fever, in order.
4) Ten cases of seventeen patients(59%) had complete resection and seven patients(41%) had partial excision or biopsy only.
The average diameter of tumor was 10.2 cm(range : 2~26 cm).
5) Three grading system based on histopathologic findings were applied in all patients: 7 cases in Grade¥°, 6 in Grade¥±, and another 4 in Grade¥².
6) Through the follow-up for 6 months to 8.2 years, overall 5-year cumulative survival rate was 59%. Five-year survival rate in ten curative and seven palliative resection cases was 90% and 14%, respectively. Four of ten curative resection
showed
recurrence(40%).
7) The corresponding cumulative 5-year survival rate in Grade¥°, ¥±and¥²was 50%, 25% and 38%. Five-year survival rates according to the tumor size were 63% in less than 10 cm diameter and 50% in 10 cm or greater. Lymph node metastasis showed
unfavorable 5-year survival rate (15% in positive metastasis, 75% in negative metastasis).
Based on the above results, tumor size, histologic grade, lymph node metastasis, and type of operation had prognostic implications, and last two factors showed stastistical signigicance(p<0.01).
